what i did was buy a car adapter usb one and just take it apart and run the wire leads to the battery. If you get a good car adapter it will already have a fuse in it. the car adapters take 12v down to 5v which is standard USB /phone charging. is yours just rated for 5 v?
if so you probably fried it because you ran 12v (battery) to it. mine works fantastically never had a problem with it. I then wired it all the way up to the frame of my bike. It also has an additional USB port to attach something other than my phone.
