I have the same issue with the same gear and it is intermittent, however the bike is moving when I try to downshift, the clutch cable is properly adjusted.

The bike was bought used from a dealer so I do not know if the clutch or other assemblies were abused.

Any ideas?

When it won't shift try letting the clutch out and then try it, they hang up for what ever reason sometimes.

It's really weird, seems to be more susceptible after coming off a high speed interstate run but has done it a few times in stop and go traffic, then it'll run perfect for a week.

The only real change I've made lately was to run the Honda 10w-40 oil this time instead of Yamalube (I try to throw the local shops some business during these hard times) and have added just a little bit more oil to make up for my misinterpretation of the dip stick not secured while checking oil procedures. No strange noises except occasional colorful metaphors coming out of my mouth when stuck in second at a stop. A buddy of mine suggested changing the oil one more time in case this change was done with a bad lot #.
